WAIVER FORM

PARTICIPANT (NAME as shown below and hereinafter called the Participant) agree to defend, indemnify and hold harmless with The Holland Museum Civil War Muster, its agents, representatives, employees, officers and directors form all damages including attorneys' fees and litigation expenses, (specifically including The Holland Museum Civil War Muster) attorneys' fees and litigation expenses reasonably necessary to successfully enforce this indemnification provision against the Participant named below resulting from or relating to any claims, actions, suits or proceeding in law, equity, or otherwise whether in negligence, tort, contract or otherwise asserted against The Holland Museum Civil War Muster, its agents, representatives, employees, officers and directors caused by or resulting from the Participant's actions and/or negligence while attending the Civil War Muster for which this waiver is signed. This right to indemnify includes and claims, actions, suits or proceedings based upon The Holland Museum Civil War Muster's own negligence.

SAFETY The Participant shall take all reasonable precautions for the safety of, and shall provide all reasonable protection to prevent damage, injury or loss to The Holland Museum Civil War Muster employees, all other persons, material, equipment, and/or property at the site or adjacent thereto. The Participant shall comply with all applicable Holland Museum Civil War Muster Guidelines and is responsible to comply with all State and Federal Safety Laws and Regulations.
